Two third of all cases ofendophthalmitis occurafter surgery. 90% are caused by bacteria andthe remaining 10% byfungi, viruses, and parasites. Incidencereported in the literatureis 0.1% to 0.4%. Though no study isavailable locally, incidence inour setup seems to be evenhigher.Traditionally endophthalmitis had been treated with topicalandsystemic antibiotics given both orally as well as parenterallybutwith a poor therapeutic response. Another mode of treatmentthathas become the standard treatment for endophthalmitis isanintravitreal injection of antimicrobials. Studies have proventhisto be an effective, probably the only effective treatmentavailableso far.The authors carried out a study on 56 eyes diagnosed as casesofendophthalmitis.5 The patients were treated withintravitrealinjections. Results were encouraging. Anatomicalintegrity waspreserved in 90% of cases and 60% had a visual acuityof 6/60 orbetter. A gold medal winning paper was presented by oneof theauthors in Ophthalmo 96 based on the above study. Greatenthusiasmwas shown about the technique. The Chairman of theconferenceadvised publishing the technique.5Endophthalmitis Vitrectomy Study (EVS) was completed in 1995. Ithada great impact on endophthalmitis management recommendations.Thestudy standardized the protocol of post cataractsurgeryendophthalmitis.6The aim of this booklet is to present in a simple way formanagementof endophthalmitis using the technique of intravitrealinjections.Secondly, we want to decrease the undue hesitancy andfear about theuse of intravitreal injections.
