With CMT Neighbours youcan now take control of their health with these powerful HealthTools: • Customize CMT Neighbours with tools and resources thatmatch your condition and your goals • Store your medication andtreatment instructions, and receive reminders to keep you on trackwherever you are • Record your treatment and other healthactivities between visits to your doctor so you can provide themwith the most accurate information • Track your symptoms, diet,exercise, mood and general condition over time to discover the carestrategies that work best for you • Link your Community Assistantsand other supporters into your own personal mobile social networkto support you on your health journey • Anonymously contributelearning from your story to a vital data resource scientists canuse to improve future patient care • CMT Neighbours is free for allresidents of St. James Town who want to play a more active role intheir health and self-care, and CMT Community Assistants who mayhelp you with your goals.
